Some families, hospital discharge planners and nursing homes are solving two problems with one solution. Transferring an older person from care setting to another care setting or just back home can be done in a cost effective manner while assuring a person to person contact that takes the fear and frustration out of the move.
We have been asked to drive clients from hospitals to nursing homes instead of putting the individual in an ambulance or van. The ride is less expensive, either to the home or to the government payor and the client is met by a trained concierge who makes the process familiar instead of fearful.
