There is often fragmentation of logistics, forms, care instructions and transport as elders transition between healthcare settings. Aging Well magazine reports that, as patients transfer from one level of care to another, they are often left inadequately prepared. For instance, they may be suddenly thrust in a position of independence after having been dependent on others for a long period of time, or vice versa.Elder Concierge Services is one of the first companies in Colorado to offer transitional services for non-medical issues and logistics. We will be there to assist with admission or discharge planning, transportation services, packing up belongings, picking up prescriptions or stocking the refrigerator for a client’s return home. We can coordinate with treatment facilities or at-home care providers to make sure everything is in order. Most importantly, we don’t rush. We take all the time we need to make sure our clients are comfortable – and comforted.

ECS offers a full range of non-medical services for seniors in transition.- On the way to a facility, we help our clients get dressed, stop for lunch, accompany them to their new setting, and make sure they are settled.
- We visit them to assure a positive transition.
- On the way home, we pack up belongings, fill out discharge papers, run any errands, accompany them home and make sure they are comfortable and settled.
- We keep families updated on the status and well-being of their loved ones at all times.
